O fibrinog&ecirc;nio destaca-se como o <strong>fator de coagula&ccedil;&atilde;o mais abundante no plasma<\/strong>, desempenhando um papel crucial na <strong>agrega&ccedil;&atilde;o plaquet&aacute;ria<\/strong> e na <strong>forma&ccedil;&atilde;o de fibrina<\/strong> para o processo hemost&aacute;tico.<p><strong>Convido-os a juntar-se a n&oacute;s do Estrat&eacute;gia MED<\/strong> para aprofundar nosso entendimento sobre o processo de hemostasia e entender como o fibrinog&ecirc;nio exerce sua influ&ecirc;ncia. Circula a uma concentra&ccedil;&atilde;o de aproximadamente <strong>200 a 400 mg\/dL<\/strong>, sendo o fator de coagula&ccedil;&atilde;o mais abundante no plasma.<\/li>\n<\/ul><ul class=\"wp-block-list\">\n<li><strong>Meia-vida<\/strong>: a meia-vida do fibrinog&ecirc;nio &eacute; de aproximadamente<strong> tr&ecirc;s a quatro dias<\/strong>.&nbsp;<\/li>\n<\/ul><h2 class=\"wp-block-heading\" id=\"h-o-processo-de-hemostasia\"><span class=\"ez-toc-section\" id=\"O-Processo-de-Hemostasia\"><\/span>O Processo de Hemostasia<span class=\"ez-toc-section-end\"><\/span><\/h2><p>A hemostasia &eacute; o processo que controla os sangramentos ocasionados por les&atilde;o dos vasos. <strong>Esse mecanismo &eacute; divido em 3 partes<\/strong>:&nbsp;<\/p><ul class=\"wp-block-list\">\n<li><strong>Hemostasia prim&aacute;ria<\/strong>: a hemostasia prim&aacute;ria, a partir da <strong>ades&atilde;o<\/strong>, <strong>ativa&ccedil;&atilde;o e agrega&ccedil;&atilde;o <\/strong>plaquet&aacute;ria, culmina na forma&ccedil;&atilde;o do plug plaquet&aacute;rio.<\/li>\n<\/ul><div class=\"wp-block-image\">\n<figure class=\"aligncenter size-large\"><img decoding=\"async\" width=\"1024\" height=\"375\" src=\"https:\/\/med.estrategia.com\/portal\/wp-content\/uploads\/2023\/11\/1-2-1024x375.jpg\" alt=\"\" class=\"wp-image-52232\" srcset=\"https:\/\/med.estrategia.com\/portal\/wp-content\/uploads\/2023\/11\/1-2-1024x375.jpg 1024w, https:\/\/med.estrategia.com\/portal\/wp-content\/uploads\/2023\/11\/1-2-768x281.jpg 768w, https:\/\/med.estrategia.com\/portal\/wp-content\/uploads\/2023\/11\/1-2-800x293.jpg 800w, https:\/\/med.estrategia.com\/portal\/wp-content\/uploads\/2023\/11\/1-2.jpg 1029w\" sizes=\"(max-width: 1024px) 100vw, 1024px\" \/><figcaption class=\"wp-element-caption\">Representa&ccedil;&atilde;o da hemostasia prim&aacute;ria.<\/figcaption><\/figure><\/div><ul class=\"wp-block-list\">\n<li><strong>Hemostasia secund&aacute;ria<\/strong>: a hemostasia secund&aacute;ria fortalece esse plug com a deposi&ccedil;&atilde;o de fibrina, a partir da cascata de coagula&ccedil;&atilde;o, formando um plug hemost&aacute;tico secund&aacute;rio ou trombo.<\/li>\n<\/ul><div class=\"wp-block-image\">\n<figure class=\"aligncenter size-full\"><img decoding=\"async\" width=\"732\" height=\"563\" src=\"https:\/\/med.estrategia.com\/portal\/wp-content\/uploads\/2023\/11\/2-1.jpg\" alt=\"\" class=\"wp-image-52233\"><figcaption class=\"wp-element-caption\">Cascata de coagula&ccedil;&atilde;o.<\/figcaption><\/figure><\/div><ul class=\"wp-block-list\">\n<li><strong>Hemostasia terci&aacute;ria<\/strong>: a hemostasia terci&aacute;ria &eacute; o processo de dissolu&ccedil;&atilde;o dos trombos de fibrina formados pelos processos de hemostasias prim&aacute;ria e secund&aacute;ria, garantindo a permeabilidade do sistema vascular.<\/li>\n<\/ul><div class=\"wp-block-image\">\n<figure class=\"aligncenter size-full\"><img decoding=\"async\" width=\"567\" height=\"582\" src=\"https:\/\/med.estrategia.com\/portal\/wp-content\/uploads\/2023\/11\/3-2.jpg\" alt=\"\" class=\"wp-image-52234\"><figcaption class=\"wp-element-caption\">Etapas da hemostasia.<\/figcaption><\/figure><\/div><h2 class=\"wp-block-heading\" id=\"h-funcao-do-fibrinogenio-nbsp\"><span class=\"ez-toc-section\" id=\"Funcao-do-Fibrinogenio\"><\/span>Fun&ccedil;&atilde;o do Fibrinog&ecirc;nio&nbsp;<span class=\"ez-toc-section-end\"><\/span><\/h2><p>O fibrinog&ecirc;nio tem in&uacute;meras intera&ccedil;&otilde;es funcionais e desempenha um papel fundamental no equil&iacute;brio hemost&aacute;tico:&nbsp;<\/p><ul class=\"wp-block-list\">\n<li><strong>forma&ccedil;&atilde;o de fibrina<\/strong>: o fibrinog&ecirc;nio atua no final da cascata de coagula&ccedil;&atilde;o (via comum), sendo convertido pela trombina (fator IIa) em fibrina para formar o co&aacute;gulo de fibrina.<\/li>\n<\/ul><ul class=\"wp-block-list\">\n<li><strong>Agrega&ccedil;&atilde;o plaquet&aacute;ria<\/strong>: o fibrinog&ecirc;nio se liga &agrave;s plaquetas e facilita a agrega&ccedil;&atilde;o plaquet&aacute;ria, embora a agrega&ccedil;&atilde;o plaquet&aacute;ria tamb&eacute;m possa ocorrer na aus&ecirc;ncia de fibrinog&ecirc;nio (por exemplo, via fator de von Willebrand).<\/li>\n<\/ul><ul class=\"wp-block-list\">\n<li><strong>Inser&ccedil;&atilde;o placent&aacute;ria<\/strong>: durante a gravidez, o fibrinog&ecirc;nio desempenha um papel na manuten&ccedil;&atilde;o da integridade da inser&ccedil;&atilde;o placent&aacute;ria.<\/li>\n<\/ul><h2 class=\"wp-block-heading\" id=\"h-disturbios-do-fibrinogenio-nbsp\"><span class=\"ez-toc-section\" id=\"Disturbios-do-Fibrinogenio\"><\/span>Dist&uacute;rbios do Fibrinog&ecirc;nio&nbsp;<span class=\"ez-toc-section-end\"><\/span><\/h2><p>Os n&iacute;veis plasm&aacute;ticos circulantes de fibrinog&ecirc;nio aumentam com a idade, obesidade, tabagismo e estados inflamat&oacute;rios e diminuem com o consumo de &aacute;lcool. Os dist&uacute;rbios do fibrinog&ecirc;nio podem ser heredit&aacute;rios ou adquiridos e podem envolver anormalidades na quantidade de fibrinog&ecirc;nio (defeito quantitativo) ou anormalidades na fun&ccedil;&atilde;o da mol&eacute;cula de fibrinog&ecirc;nio (defeito qualitativo).&nbsp;<\/p><h3 class=\"wp-block-heading\" id=\"h-disturbios-quantitativos\">Dist&uacute;rbios Quantitativos<\/h3><ul class=\"wp-block-list\">\n<li><strong>Cong&ecirc;nito<\/strong>: Afibrinogenemia (autoss&ocirc;mica recessiva) e hipofibrinogenemia (autoss&ocirc;mica dominante);<\/li>\n<\/ul><ul class=\"wp-block-list\">\n<li><strong>Adquirido<\/strong>: Hipofibrinogenemia (por exemplo, doen&ccedil;a hep&aacute;tica, terapia de plasmaf&eacute;rese, coagulopatias consumptivas como CIVD), hiperfibrinogenemia (por exemplo, inflama&ccedil;&atilde;o, neoplasia, gravidez) e medicamentos (por exemplo, agentes fibrinol&iacute;ticos, isotretino&iacute;na, L-asparaginase, tigeciclina e &aacute;cido valproico).<\/li>\n<\/ul><h3 class=\"wp-block-heading\" id=\"h-disturbios-qualitativos\">Dist&uacute;rbios Qualitativos<\/h3><ul class=\"wp-block-list\">\n<li><strong>Cong&ecirc;nito<\/strong>: disfibrinogenemia (autoss&ocirc;mica dominante) e hipodisfibrinogenemia (autoss&ocirc;mica dominante);<\/li>\n<\/ul><ul class=\"wp-block-list\">\n<li><strong>Adquirido: <\/strong>doen&ccedil;a hep&aacute;tica, malignidades (por exemplo, carcinoma renal, hepatoma, mieloma m&uacute;ltiplo) e anticorpos antifibrinog&ecirc;nio.&nbsp;<\/li>\n<\/ul><h2 class=\"wp-block-heading\" id=\"h-cai-na-prova-nbsp\"><span class=\"ez-toc-section\" id=\"Cai-na-Prova\"><\/span>Cai na Prova&nbsp;<span class=\"ez-toc-section-end\"><\/span><\/h2><p>Acompanhe comigo uma quest&atilde;o feita pela Universidade Federal de Pernambuco &ndash; UFPE <strong>(dispon&iacute;vel no banco de quest&otilde;es do Estrat&eacute;gia MED):&nbsp;<\/strong><\/p><p><em>No tromboembolismo pulmonar &eacute; observada redu&ccedil;&atilde;o na concentra&ccedil;&atilde;o circulante de:&nbsp;<\/em><\/p><p><em>A. troponina I&nbsp;<\/em><\/p><p><em>B. d&iacute;mero D<\/em><\/p><p><em>C. &aacute;cido &uacute;rico<\/em><\/p><p><em>D. fibrinog&ecirc;nio.<\/em><\/p><p><strong>Coment&aacute;rio da Equipe EMED:<\/strong> o fibrinog&ecirc;nio &eacute; um dos fatores da coagula&ccedil;&atilde;o sangu&iacute;nea (fator I), uma prote&iacute;na necess&aacute;ria para a forma&ccedil;&atilde;o do co&aacute;gulo. Produzido no f&iacute;gado e liberado na circula&ccedil;&atilde;o junto com outros fatores, &eacute; estimulado quando h&aacute; les&atilde;o de um vaso sangu&iacute;neo, processo chamado hemostasia, que cont&eacute;m o sangramento. As plaquetas aderem ao local da les&atilde;o e se agregam formando um tamp&atilde;o. Ao mesmo tempo, &eacute; iniciada a cascata da coagula&ccedil;&atilde;o. Ao final do processo, o fibrinog&ecirc;nio, uma prote&iacute;na sol&uacute;vel, &eacute; convertido em filamentos insol&uacute;veis de fibrina, que refor&ccedil;am o co&aacute;gulo plaquet&aacute;rio e formam um co&aacute;gulo est&aacute;vel. Da&iacute; o motivo de sua diminui&ccedil;&atilde;o no contexto do TEP agudo.<strong> Portanto, alternativa D.&nbsp;<\/strong><\/p><p>Venha fazer parte da <strong>maior plataforma de Medicina do Brasil<\/strong>!
